- The report mentioned handwashing facilities were inadequate.
- The report flagged unclean non-food contact surfaces.
- Equipment, utensils, and linens were not appropriately stored.
- Plumbing fixtures, backflow devices, and drainage systems were not maintained.
- Floors, walls, and ceilings were not maintained, the report noted.

- Ventilation and lighting were not maintained. The designated areas were not used appropriately.
