Located in Nashville, Till Five Pizza serves pizzas made with hand-tossed dough and locally sourced ingredients. According to the inspection report, the violations are as follows:
- The kitchen staff lacked adequate knowledge of basic food safety practices, including proper glove use and storage separation between raw and ready-to-eat foods.
- An employee was seen putting on gloves without washing their hands before handling raw beef, the inspection report stated. Proper handwashing procedures and the appropriate use of gloves by staff were discussed as a corrective action.
- The report noted that raw beef patties were stored above ready-to-eat pasta and marinara sauce in a prep cooler. The items were rearranged, and staff were trained on correct storage practices for raw and RTE foods.
- Raw shelled eggs were stored above cheeses and containers of diced pineapples in the walk-in cooler. The items were rearranged with additional staff training.
- The walk-in cooler air temperatures were between 44°F and 46°F, the inspection report stated. Cheeses were moved to a prep cooler that maintained a proper temperature. A 24-hour notice was issued for the walk-in cooler.
- Several food items, including shredded lettuce, sliced tomatoes, and chicken wings, were stored without date labels in the prep cooler. The person in charge stated the items were prepped the previous evening, the report said. As a corrective action, the items were labeled, and a 24-hour date-marking requirement was relayed to the staff.

- Bulk containers of sugar and flour were stored without labels in dry storage, according to the report.
- Containers of food items were stored uncovered in the reach-in freezer.
- A wet wiping cloth was kept on prep surfaces near the prep cooler.
- A white, mold-like buildup was seen on racks inside the walk-in cooler.
- The dumpster area had a large accumulation of trash surrounding it, the report said.
- The report pointed out that the most recent inspection was not posted as required.
