The inspectors observed inadequate handwashing facilities at Deja Vu Pizza & Pasta. The sink near the cook’s line was blocked, and the paper towel dispenser was not in good condition. The facility was storing potentially hazardous foods (PHF) cold at temperatures higher than the recommended 41°F or lower.

Inspectors noted that no sanitizer solution at 100 ppm chlorine was available during food preparation. Cutting boards on all food contact surfaces should be thoroughly sanitized. The facility was heavily infested with rodents.

Inspectors found droppings on the cooking range, beneath the pizza prep cooler, on top of ingredient containers, and throughout other key areas. The facility should maintain proper waste disposal practices using leak-proof and rodent-proof containers.

Follow-up Inspection

Before requesting a reinspection, the restaurant must eliminate rodent activity through approved methods and sanitize affected areas.

After the inspection, the San Francisco Department of Public Health, Environmental Health Branch, closed Deja Vu Pizza & Pasta. Before reopening, the restaurant must submit a pest control report.

The facility should ensure all violations warranting the closure are resolved. A department official may only remove the closure placard.
