Three San Francisco Restaurants Temporarily Closed After Routine Health Inspection

The San Francisco Department of Public Health, Environmental Health Branch, reported several food code violations at three restaurants on Monday, June 16. The inspections resulted in the closure of the establishments due to non-compliance with health standards.

Ramen Hiroshi

The most serious violation reported by the inspectors at Ramen Hiroshi was evidence of rodent infestation. Inspectors noted rodent droppings in the main kitchen, near the entrance, and around the walk-in cooler. Inspectors observed an employee attempting to wash gloved hands without soap.

Several points of entry of vermin were also identified through gaps in doors and in a brick wall of the storage room. Other significant observations during the inspection revealed a hand sink in the downstairs kitchen obstructed by a refuse bin and grease and debris accumulated on the filters of two ventilation hoods.

The restaurant was also missing quaternary ammonium test strips. Some of the violations were corrected on-site including fish thawing at ambient temperature and incorrect cold holding temperatures. The health inspection is published online.

Haystack Pizza Restaurant

According to the report, the facility failed to use a sanitizing solution before commencing food preparation. Standing gray water/raw sewage was observed by the inspectors in the garbage areas where improper food was stored. The facility is required to clean and sanitize the affected areas immediately.

Inspectors found evidence of rodent activity in various areas of the facility. Numerous mouse droppings were observed on the front bar counter, in the rear dining room, underneath the equipment in the kitchen, and inside the non-working refrigerator among others.

Gaps were found where the wall meets the floor in the kitchen and at the front counter. All holes and gaps must be sealed using rodent-proofed materials; use of spray foam is unauthorized.

Inspectors reported a buildup of condensation inside the walk-in cooler. The lack of paper towels in the handwashing sink in the kitchen was corrected on-site.

Rodent droppings were identified on the floor and shelving throughout the facility including the upstairs dry storage area. The report mentioned that the restaurant is required to provide proof of pest control service. Grease and debris were accumulated in the service panel of the deep fat fryer and fan guards in the walk-in box.

A mold-like residue was observed on the plastic vegetable crates. Multiple concerns related to the upstairs employee bathroom revealed the absence of soap, paper towels, and a functioning toilet handle.

Cleanliness and sanitation issues need to be addressed by June 17 including lack of sanitizer solution during food preparation and unclean food contact surfaces. The facility should ensure it is professionally vermin-proof and seal all potential entry points.

Re-inspection Requirements

As a result of the failed health inspection, CLOSED placards were issued and posted outside all three restaurants. The placard should not be obstructed and may only be removed by an inspector of the department. The permit remains temporarily suspended which means the facilities should cease all food handling.

The restaurants shall remain closed until all conditions warranting the closure are corrected and the department has reinstated the permit. Each facility is required to address all the violations before requesting a reinspection. Failure to comply with the closed notice may result in a possible fine of $1,000.00 and/or imprisonment for not more than six months for each offense.
