Mountain Mike’s Pizza, a fast-food chain, was cited for multiple violations of the health code. The health inspectors saw a glass cup containing an employee’s drink on the food preparation table. Inspectors also found potentially hazardous foods (PHFs) stored at unsafe temperatures.
Cooked wings were kept in the walk-in cooler at 44 degrees Fahrenheit. Cooked ham/pineapple pizza was kept in the hot holding unit at 130 degrees Fahrenheit. The employees were asked to store all PHFs at 41 degrees Fahrenheit or below for cold holding and 135 degrees Fahrenheit or above for hot foods.
Inspectors reported old food residue on the surfaces of the can opener and a buildup of syrup and residue on the soda dispenser nozzles and surrounding surfaces.
The report said multiple German cockroaches, including nymphs and adults, were observed throughout the facility, indicating an active infestation.
The inspectors saw two live roaches behind the two-compartment sink, two dying roaches in a glue trap located beneath the two-compartment sink, three dying roaches under the warewashing machine, one live roach behind the refrigerator near the main oven, and two dying roaches and multiple unidentified bugs in two separate glue traps near flour bags.
Four dough portions were observed on the food preparation table, left unattended and uncovered during active preparation. Several areas within the facility were found to be unclean or improperly maintained, posing potential contamination risks.

Follow-up Inspection
Inspectors advised the restaurant to use licensed pest control services to address the infestation. Once the violations are corrected, the restaurant can contact the Environmental Management Department to schedule an appointment for reinspection.
The restaurant must eliminate the vermin infestation to have its health permit reinstated and avoid continued closure.
