Out of Wack Jacks Bar & Grill is a sports-centric tavern in Philadelphia. Residents and visitors enjoy sports and game nights at the spot with delectable food. In the inspection conducted on Friday, the outlet was found to have multiple cleanliness and maintenance issues.

The report stated that the lighting under the hood system was non-functional. The hot water faucet was leaking, the mop was not stored in the mop bucket, and debris and rusted shelving were observed. Black residue and pink slime were observed on the interior of the ice machine bin around the kitchen area. Cutting boards had deep scratches and scoring, not allowing for effective cleaning and sanitizing.

Visible physical evidence of rodent activity was observed in the kitchen area, as could be seen in the mouse droppings. Food temperature-measuring devices were used, but they were not properly calibrated.

Proper written procedures or HACCP plans were missing to prepare meat foods using reduced oxygen packaging. Several ready-to-eat foods prepared in the food facility were held for more than 48 hours and were not date-marked. All the violations are mentioned in the official report.

Kebab USA is a go-to spot for people to enjoy kebabs and Mediterranean dishes. Just like usual inspections at restaurants, a health inspection was conducted at Kebab USA on March 28, and the results were unsatisfactory. Violations of code were observed; some were corrected on-site, and some were ordered to be fixed by the restaurant.

The report mentioned a food safety-certified person was not at the facility and came 20 minutes after the start of the inspection. The chicken was kept uncovered in walk-in cooler units. The meat slicer had hard, old food debris. Sink stoppers were not working properly, and air pods were found lying on the cutting board of the cold prep unit.

The report also stated that the food in the bulk containers was not labeled with their common-use names. One fruit fly was observed in the front service area. Oil cases were stored on direct floor surfaces and not at least 6 inches above the floor. Instead of a commercial microwave, a domestic microwave was observed in the kitchen area.

Aluminum foil was used to cover the cooking equipment. Grease accumulation was observed on the cooking equipment. Dusty cover vents were observed in the men’s and women’s restroom areas. The two restaurants were not in compliance with food safety regulations and are required to eliminate the violations. The compliance status will be assessed upon reinspection by the authorities. If any of the restaurants fails to do so, it may result in the revocation of the health license and other legal actions.
