Health Inspectors Recommend Closure of San Francisco Restaurant After Violations

Vietnamese restaurant Lam Hoa Thuan went through a routine inspection on November 25 by the San Francisco health department. The eatery was cited for multiple violations, including dead and live roaches and grease accumulation across the prep area.

Lam Hoa Thuan is a Vietnamese restaurant that also serves Chinese and Cantonese food. Menu items include roast duck, BBQ pork (char siu), and soy-sauce/ginger-style chicken.

According to the inspection report, the restaurant was ordered to shut down temporarily after it was cited for several violations. Violations cited included:

  • The report said the facility was operating an unauthorized outdoor food prep station without a nearby hand sink. A worker was seen preparing raw turkeys in that area and used a disposable glove kept on a shelf.
  • The report said the front-counter paper towel dispenser was empty, though it was replaced during inspection. There were no paper towels at the tea station, and the main prep hand sink had no soap in its installed dispenser. They were using a pump bottle as a substitute.
  • The report noted the dish machine failed to dispense chlorine at detectable levels and recommended repair to reach roughly 50 ppm. It also called for proper sanitizer solutions at all workstations.
  • The report said rat droppings were found under a prep table with a meat grinder and in the customer dining room near the hallway to the restrooms.
  • A dead roach was observed in the sink by the dishwasher. There were also dead and live roaches near the to-go containers stored above customer utensils and the rice warmer. Pest control invoices from August and September 2025 were provided. The PIC noted that the treatments have been taking place weekly.

  • The report noted bins of raw meat stored directly on the floor and beneath plumbing fixtures. It also said that outdoor food prep exposed products to environmental contamination.
  • Inspectors observed heavy grease buildup throughout the back prep area, the report said. It included soiled shelving and prep tables.
  • The report said the dish machine required repair and that the two-door prep-top refrigerator must be fixed to maintain temperatures at or below 41 °F.
  • The report noted missing or broken floor tiles in the back prep area and called for their replacement.

According to the official report, the restaurant received a red placard, which led the eatery to shut down. The eatery is expected to have a professional cleaning company to deep-clean the entire facility, followed by pest control service. It can request the officials from the health department for a reinspection.
