[Editor’s note: The reports referenced in this article reflect conditions observed during inspections conducted on March 9 and do not represent the establishments’ current or future status.]
The San Francisco Department of Public Health conducted routine inspections at local restaurants on March 9. Fishmandu Sushi and The Willows were recommended to shut down due to major violations.

Fishmandu Sushi

- When: March 9, 2026
- Where: 846 Geary St, San Francisco, CA 94109
Fishmandu Sushi serves sushi and seafood, along with a few American dishes. According to the report, the violations reported were:
- The person in charge (PIC) was observed washing their hands with water only after interacting with a salesperson at the front door and handling their phone while showing the inspector the manager’s food safety certificate. The PIC later washed their hands properly.
- Cockroaches were seen on plates, prep top, prep table, under the counter, on shelves near the prep station, and on clean dishes near the three-compartment sink, the report said.
- The warewashing machine failed to produce detectable sanitizer, as chlorine test strips showed no sanitizer after two cycles.
- Multiple cockroaches were seen at the handwashing sink, above the three-compartment sink, in dishes next to the sink, on pipes near the floor beside a one-door refrigerator, under the counter near the sushi bar, and under plates stored above the prep station.
- Cockroach droppings were also observed on a ceiling conduit in the kitchen, along the wall near the office by the stairs, on ceiling pipes near the upstairs area, and under the counter near the point-of-sale area, the report mentioned.

- A can of raid and a can of boric acid roach powder were found in the facility. Only a state-licensed pest control operator may apply pesticides.
- Refrigeration equipment was found with torn and damaged door gaskets.
- The report noted that a bed and clothing were found in the upstairs dining area, which appeared to be set up as a living space. The person in charge stated the items would be removed within a few days.

The Willows

- When: March 9, 2026
- Where: 1582 Folsom St, San Francisco, CA 94103
The Willows is a gastropub that serves craft beer and cocktails. According to the report, the report documented several violations.
- An employee was observed wiping their hands on a dry wiping cloth instead of properly washing them. The employee was asked to wash their hands, which they did, and the wiping cloth was relocated for sanitizing.
- Residue was observed on the ice deflector panel inside the ice machine.
- The report noted that live cockroaches were seen on a sticky trap on the floor behind the reach-in freezer near the kitchen entrance, on piping beneath the dishwashing area, and in the back bar area under the drip tray among old, wet coasters.
- Cockroaches were found on a sticky trap behind the cooler in the back bar. A dead cockroach was also observed stuck to tape above the kitchen handwashing sink.
- Several cockroach droppings were found on the piping below the dishwashing area and under the counter at the back bar. Cleaning began during the inspection, and pest control was contacted and scheduled to service the facility.

- The report noted heavy liquid accumulation on the floor behind the ice machine in the back storage area.
- Chipped paint on the wall was observed next to clean bins above the three-compartment sink, and cracks or chips in the flooring along the main cookline in the kitchen.
- At the main bar’s three-compartment sink, the hot water faucet dispensed cold water, and the cold water faucet dispensed hot water. The faucets must be switched or properly relabeled to prevent a potential safety hazard.

Follow-up Inspection

According to the reports, both establishments received temporary permit suspensions. They have been recommended to use proper pest control methods and to correct the violations before the reinspection.
