During an inspection on March 10, the San Francisco Department of Public Health reported multiple violations at Lers Ros 16th Street. The report states the facility was recommended for temporary closure.

Lers Ros is a Thai restaurant is known for serving traditional Thai cuisine. According to the report, the violations noted were:
- No soap or paper towels were available at the only handwash sink in the food preparation area, and utensils were stored in the sink basin. Soap and paper towels were provided, and utensils were removed during the inspection.
- Two containers of tofu stored in the prep cooler measured 44°F, above the required 41°F or below. The containers were relocated to the interior of the cooler.
- Two containers of curry were found cooling at room temperature with a temperature of 60°F. The curry was placed in an ice bath to continue the cooling process.
- Live and dead cockroaches and cockroach feces were in multiple areas, including a commercial microwave, shelving above the handwash sink, and the POS ticket printer shelf. They were also found in an electrical closet, beneath the three-compartment sink, near the grease trap, at the front service station, near sugar bags, and under a household microwave.

- Food containers were stored in unapproved areas, including beneath the clean side drainboard of the dish machine and under the handwash sink.
- The report states that food was cooked in an unenclosed area. The restaurant must discontinue this practice, according to the report.
- A household pesticide can was found above the mop sink, the report mentioned.
- Cooking equipment had a heavy buildup of grease and required deep cleaning. A household-use Panasonic microwave was present in the facility and must be removed, as only commercial-grade equipment is allowed, the report said.
- Missing base coving tiles were observed under the exhaust hood canopy, and floors under cooking equipment required deep cleaning.

Follow-up Inspection

According to the official report, the restaurant had its health permit suspended during the inspection. A follow-up inspection is expected on or after March 13, according to the report.
