When: June 30, 2026
Where: 25606 Crown Valley Pkwy Suite# K-1B, Ladera Ranch, CA 92694, United States
Thai Fix is a family-owned Thai restaurant. The inspection report documented the following violations:
- The report noted that the handwashing sink lacked supplies and had an empty soap dispenser and no paper towels. The staff was instructed to provide soap and paper towels in the handwashing sink and maintain all handwashing sinks as per the requirement.
- The restaurant stored knives on an unclean and rusted magnetic knife holder. Cutting boards were kept in contact with the inside portion of the handwashing sink. During the inspection, the manager removed the knives and cutting boards from the magnetic holder to wash, rinse, and sanitize them. The staff was instructed to maintain all food contact surfaces and prevent contamination.
- The report noted the floor sinks and drains overflowing when the three-compartment sink was drained at full capacity. The staff was instructed to repair and maintain all plumbing and plumbing fixtures, which must be fully operative, clean, and in good repair.
- According to the report, live cockroaches behind the magnetic knife holder above the three-compartment sink. Rodent droppings were also found behind the sink, around the water heater, and in the storage area outside the restroom.
- The restaurant was directed to eliminate the cockroach and rodent infestation using approved pest control methods. The staff was instructed to thoroughly clean and sanitize all affected surfaces.

- The report stated that the containers holding food or food ingredients that were removed from their original packages for use in the food facility were not labeled properly. Food containers must be clearly labeled for easy identification.
- Accumulated grease and debris between the stove and the wok range were also noted. The restaurant is required to remove the accumulation.
- The restaurant was reusing cardboard to line shelving in the food prep area and walk-in cooler. According to the report, inspectors instructed the restaurant to discontinue the practice and replace the cardboard with smooth, durable, and easily cleanable materials.
- The magnetic knife rack was heavily rusted and broken and must be removed and replaced. Inspectors found scoop handles resting directly in food ingredients. The report instructed storing scoops in a sanitary and cleanable container away from the ingredients.
- The restaurant has stored utensils without inverting them on the order preparation counter, leaving the food contact surfaces exposed. The utensils are required to be stored inverted. Knives, forks, and spoons must be stored or served so that only the handles are touched by employees or customers.
- An employee’s open drink was stored on the cutting board portion of the cold top. The staff was instructed to store all personal employee items in a separate, designated area and provide a lid and straw to all employee drinks.
