The inspector noted that the facility had severely dented, swollen, and distressed canned items in the walk-in cooler area. Inspectors saw an open beverage belonging to an employee in a food prep area. Uncovered food was observed in the chest freezer during the inspection.

The chlorine concentration in the sanitizing solution was measured at 0 ppm, rather than 300-500 ppm as required. Inspectors observed scum or film on the handwashing sinks, prep sink, and three-basin sink, indicating they were not properly cleaned and sanitized. Containers in the reach-in refrigerator and cold prep were not labeled with the name of their contents.

Inspectors found evidence of roach activity near the basement air gap. The facility’s outer openings do not prevent insects, rodents, or other animals from entering.
